Things are looking up! - 今年は上向きな予感。

I've had another appointment with my endocrinologist -- and guess what? My free Triiodothyronine (T3 thyroid hormone) and TSH (thyroid stimulating hormone) levels came back NORMAL. I don't remember how long it's been since I got my last "normal" thyroid test results. One (or three or four) less pill to take? I'd take that! (Now that I know a bit more, it seems my hormone imbalances mostly stem from pituitary gland -- as TSH is produced by pituitary gland's order.)

I don't know if my thyroid glands were rudely woken up by 7 weeks of not taking thyroid medications (shock therapy??), or if additional hydrocortisone is helping, but this gives me hope -- in that maybe my body doesn't have to be screwed up forever in every gland we look at.

The nagging questions remain, though -- then why do I keep hosting opportunistic, chronic infections? (I've had a sore throat since September of last year -- wait -- was it September of 2008? Not sure.) Why do I keep hurting? But asking such things might be like riding on a time machine and going all the way back to my childhood, adolescence, and younger years -- since it might have been a cumulative effect -- so I don't have to have all the answers, as long as I keep feeling better bit by bit.

Maybe it's the quality of sleep -- maybe I'm not getting enough stage 4 sleep (a.k.a. deep sleep). (In one study, healthy college-age subjects were woken up every time they entered into stage 4 sleep, thus depriving them of deep sleep. Otherwise they were allowed to sleep. After about a week of doing this, most of them developed fibromyalgia-like symptoms like widespread pain. Isn't that interesting?) Or, is my small intestine still not healed enough to absorb nutrition & turn it into energy?

The lucky thing is, Dr. Patrick Wood, former LSU professor and head of LSU Fibromyalgia Research Department and Clinic, current scientific advisor for the National Fibromyalgia Association, has come to Pacific Rheumatology Associates in Renton. (Renton is south of Seattle, between Seattle and Tacoma.) And I have an appointment with him next week! (I was in front of the line, since I got in contact with them last year, hoping to see their other doctor, Dr. Holman, who'll be concentrating on his reseach.) This rheumatology clinic specializes in and only see fibromyalgia patients. Finally, a scientific expert! Plus Dr. Wood's services are covered (as far as I know thus far) by our excellent insurance.

I had been working with Fibromyalgia and Fatigue Center in Bellevue, which is known for following their medical director, Dr. Jacob Teitelbaum's protocol. While they were helpful in getting clues to my body's various mishaps, due to high turnover in their staff, I was onto my 4th doctor (in less than 3 years) -- and that doctor recently quit.

After my 1st doctor, I wasn't confident if the following doctors could be considered experts on the subject; and my most recent doctor quitting was my final straw. Since their treatments are only partially covered by insurance (often reimbursements were miniscule), we ended up spending a lot of money there. I'm still thankful, though, because they probably kept my chronic infections at bay with all those IVs and immunogloblin shots; That meant I didn't have to reach the point of acute adrenal crisis, which I had no idea about, and I could've died from that.

...Although it is disturbing that they once considered treating me with hGH (human growth hormone), which often causes adrenal crisis in unsuspecting adrenal insufficiency patients -> comatose -> death.

Well, see, it goes to show how lucky I am. I think my grandmother's and my aunt's spirits (and many people's prayers) are protecting me. So thank you!

My sleep doc (who happens to be the director of Virginia Mason Sleep Disorders Center! He gets extra points for being married to a Mainer) is tweaking my sleep meds, so maybe that would help my sleep and stop the hand tremors I've been having. (Those of you whose emails I couldn't respond to in a timely fashion, sorry -- every time I got tired, my hand pain and tremors got worse. It took me 3.5 hrs to write this easy post.)

I have another thyroid test and an appointment with my endocrinologist in 2 weeks -- and for once I'm excited to get test results back. Will they be normal again?

I feel like things are starting to look up. Last year was a rough year (<- evident from the number of posts), because I thought I was getting better but rather got slightly worse. This year I'm getting top-notch medical care (Dr. Wood is just starting to see patients -- I couldn't have timed it better), and getting all sorts of answers. It's cloudy outside, but my heart is filled with warmth and thankfulness. :-)

And thank you, those of you close to me, (and those who prayed for me even though we're not that close) for patiently supporting me and encouraging me -- I couldn't have done it alone.

また内分泌系の専門医のところへ行ってきました。先週甲状腺ホルモンの再々再々再々再(笑)検査をするために血液を採取したのですが、なんとなんと!トリヨードサイロニン(Triiodothyronine、T3とも呼ばれる、最も強力な甲状腺ホルモンで、体温、成長、心拍数などを含めた体内のほぼ全ての過程に関与している)と、甲状腺刺激ホルモン(thyroid stimulating hormone、TSHと呼ばれ、甲状腺に働きかけ甲状腺ホルモンの分泌を促す)のレベルが、両方「正常」。これは夢???ほっぺをつねりたくなりました。薬を飲まなくていいってこと?それが一番嬉しいかも。 0(^-^)0

(考えてみたら、TSHは下垂体からの信号によって分泌されるので、いつも何かしら下垂体が関わっているみたい。お豆サイズのくせにやってくれるな、下垂体。)

7週間の薬断ちが「おいヤバいぞ」と甲状腺を叩き起こしてくれたのか(ショック療法?)、副腎関係の治療が功を奏してくれたのか分かりませんが、新たな希望が生まれました。なにか内分泌器官を診るたびに何らかのトラブルを見つける、というパターンから抜け出せるかも知れない。

でもちょっと気になることは、、、甲状腺が働いているのに、何故しつこい慢性感染が続くのか?と去年の9月からずーっと水玉模様の喉が痛い自分は考えてしまうのです (あれ?一昨年の9月だったかも・・・。)。なんで体が痛いのが続くのかも。

その答えは一生分からないかも知れません。でも雪のように昔から積もり積もって支えきれなくなって、屋根からどしゃっ、と落ちたような現象かも知れないし、タイムマシーンに乗って子供の頃からなにがあったかをつきとめるような疑問なので、全部分からなくてもいいです、少しずつ良くなっていけば。

ひとつの可能性としては、睡眠障害のせいで一番深いレベル4(ゲームみたい)の睡眠が出来ていないのかも。(ある研究では、ピチピチ健康体の大学生を集め、そのうち半分の人達に対してはステージ4の睡眠に入りそうなときにいちいち起こして、ステージ1〜3の睡眠しかとれないようにしたそうです。なんかいじわるっぽいけど。そうしたらものの1週間ほどで、起こされている学生グループのほうは、線維筋痛症のような症状(全身にわたる痛みなど)が現れたそうな。睡眠って大事なんですね〜。)

それか、セリアック病でダメージを受けた小腸がまだ回復中で、栄養が吸収されていない・エネルギーに変換されていないという可能性もあります。あーややこしい。

でも幸運なことに、パトリック・ウッド先生という、以前ルイジアナ州立大で線維筋痛症のリサーチとクリニックの主任を勤めていた医師が、ご近所のクリニック(Pacific Rheumatology Associates)にやって来るのです。公式には2月中旬からウッド先生は診察を始めます、ということになっているのですが、私は去年からそのクリニックのホルマン先生(これから研究に専念するらしい)に診てもらいたくて連絡をとっていたので、なんと来週(1月最後の週)に診ていただけるのです〜。ついに、研究を重ねてきたエキスパートに診てもらえる。しかも(分かっている限りでは)保険が使える!

前々から、線維筋痛症/慢性疲労症候群センターというところで治療を受けていたのは親しい方ならご存知の通りですが、そこは(有名・リッチになってハワイに住んでいる)ジェイコブ・タイテルバーム先生という、FMS/CFS の分野では著名な医師の治療法を実践する、というところでした。そこでさまざまなヒントは得たものの、スタッフの入れ替わりが激しく、3年足らずの間に4人の医師(と5〜6人の看護士)にかかりました。そのたびぜーんぶ説明しなければならないし(これが疲れる・・・慢性疲労症候群を良くするところ、っていうのが皮肉)、1人目のお医者様のあとはなんか私のほうがいろいろ知ってるかも、と思うような感じでした。しかも今月、4人目のドクターが辞めるというのです。

クリニックは前払いで(しかも高い)、保険会社に書類を提出してもスズメの涙のような額しか出してもらえず、ずいぶんそこでお金を使いました。それでも感謝はしています。そこでの治療のおかげでいろいろ学びましたし、慢性感染も、もっとひどくなるかも知れなかったのを、栄養・坑ウィルスの点滴やガンマグロブリンの注射などで、ひどくなる手前で抑えていてくれたのだろうと思うからです。高熱を出して急性副腎不全(副腎クリーゼ)とかになっていたら、対応策を何も知らなくてそのままあの世へ・・・ってこともあり得たことですし。

・・・でも一時は、ヒト成長ホルモン(hGH、human growth hormone)も試してみる?とか言われたこともあったっけ。。。「いえそれはなんか怖いのでやめときます」と言って良かった〜。慢性副腎皮質機能低下症+hGH って組み合わせは、多数の人が急性副腎不全 -> 昏睡 -> 死、となるらしいです。

というようなことを考え合わせると、私はとてもラッキーだったんだ〜、と思わざるを得ません。祖母と伯母が天国?から見守ってくれているのと、多くの人に祈っていただいているお陰だと思います。あらためてありがとうございます! m(__)m

私の睡眠障害専門の先生(会う前は知らなかったけれどバージニア・メイソン病院の睡眠障害科の主任だった)も睡眠導入剤をいろいろ細かく調整しようとしているので、それが深い睡眠の助けとなって、手の震えも止まるかも。(メールにすぐお返事できていない方々、申し訳ありません!最近疲れると特に手が痛くなるのと、それまでよりもっとブルブル震えてしまうんです。。。実際これを書くのに3時間半かかってしまいました。)

また再来週、薬を飲まなくてもやっていけるかどうか、甲状腺ホルモンの検査をします。テストの結果が楽しみなんて不思議!また正常反応が出るかな〜。

なんだか運気が上向きになってきた感じがします。去年は、進歩してるかな〜、と思ったところになんだか前より調子が悪くなってしまったのでつらい年でした(<- ブログ更新の数から見てもわかる)。今年はなんだか次々と一流の専門医にかかれるし(ウッド先生は患者をとり始めたばかり!ですから、なんとも完璧なタイミングでした)、答え/治療方法も出てきています。まだ1月なのに幸先がいいです(関係ないけど懸賞も2つ当たったし!)。算命学を勉強した親友みっちゃんによると、2月4日以降は晴れて天中殺があけるそうですし。

外は曇りですが、心の中はあたたかく感謝の気持ちでいっぱいです。

Miracles all around the world. - 世界中で起きてる、奇跡。

みなさん、お元気ですか~?

すっかり秋っぽくなってしまった、シアトル。
時は丑三つ時、外は言うまでもなく真っ暗な中、
時折雨足が強くなる音が聞こえます。

(更新するって言っときながら、なかなか調子の出ない
週が重なって、ごめん、みっちゃん!)

なにか小さなことでも、する度に上がったり下がったり
する体温。 この免疫不全による慢性感染ってのは、
1年前思っていたよりしぶといようです。

慢性の単核症(モノ、EBV)、HHV6ウイルスも、
私の熱と同じで、上がったり下がったりまだ健在(?)。
変に火照って寝汗で眠りにつけない夜が多く、
冷蔵庫で冷やした冷えピタ(ありがとうお母さん!)が
気持ちいい~。

でも、でも、小さな奇跡、そして大きな奇跡は次々と
身の回りで起こっていて、それが私に勇気を与えます。

例えば親友みっちゃんの赤ちゃん! (大きな奇跡。)
自分に関わりのある子にはひいき目になるとは
言うけれど、ほんとに可愛いと思う!


見てくださいよ、このつぶらな瞳!
そして赤ちゃんのくせに意思の強そうな
眉と口元。 かわいすぎる~。



大きな奇跡の続き:
すくすく育っている姪っ子オリビア。

こちらのほうも、腰がすわっているというか、
人をじーっと見つめる赤ちゃんらしいです。
なんでアメリカの赤ちゃんは毛が少ないんでしょうね~。


今度は、小さな奇跡。 今年は調子が悪かったので夏休み(?)も
ステイケーション、てなことで家に殆どおり、シアトルのダウンタウンにさえ
行ってなかったのですが、だからこそ目撃できたこともあって。



あまりいいカメラがないので見にくいかも知れませんが、
フクシャ(fuchsia)のお花です。

春に近所でハチドリさんを見かけて、育てたら来るかも!
(来なかったけど)と植えたもの。 外に出かける元気が
ない毎日夏のあいだ、お天気の日はバルコニーで
気分はアルフレスコ、とお夕飯を食べるのが日課でした。

とある日、あ、この花もうすぐ咲きそう・・・と思ったとき、
パツン、と音が聞こえたわけではないけれど、そんな感じで
弾けるように、花びらがひとつ、ふたつと開きはじめるのを見たのです。
「見てダニエル!」と叫んで、そのあと5分ほどふたりで、
お花が健気に咲く、まさにその瞬間を目撃したのでした。
みっつ、よっつ、と順番に開いていく花弁・・・そして食事を終える頃、
そのお花は全部開いて私達に微笑んでいたのです。

それはまさに奇跡。 ふたりでその日はニコニコ。
こんな共同体験って、忙しく動き回っていたらきっと、ない。

この2年、いろいろな感情を共有しているけれど、
それは幸運なことだと思います。
普通の生活では気付かないこと、体験できないこと、
いろいろあるから。

雨も落ち着いてきました。 また、寝るのに挑戦して
みようと思います。

How is everyone nowadays?

It totally seems like fall already in Seattle. It's 2:30 AM after a big game (UW) night.
The night outside is dark and quiet; I hear the rain come down harder once in a while. It's very soothing.

This summer has been a bit rough. It seemed, every little thing I did elevated my body temperature at night. This whole immune deficiency and chronic infections business has been trickier to treat than I thought -- a year ago, I was feeling all feisty and ready to beat it in a couple of months. (I'm competitive, so when they say "It takes a while for most people," I hear in my head, "Oh yeah? I bet I can beat it faster!")

The chronic Epstein-Barr (mono) and HHV6 viruses seem to be still well (?) and active in my body. The viruses and my temperatures have their ups and downs like a teenager's emotional drama. (I should maybe make a sitcom out of it.) I become weirdly sweaty and clammy at night; the cool-down patches (they should sell them here! It's a godsend) my mom sends me really come in handy, esp. when I have them chilled in the fridge. It feels so good on my forehead right about now.

But all those physical challenges aside, miracles are happening everywhere! And they provide me with joy and hope that the world is a nice place.

Exhibit A of a big miracle is the first picture above. He's my best friend Michiko's first baby! How adorable is he? I know, I know, they say we all become partial to the kids we are related to or associated with, but he is genuinely positively darling. I love his willful eyebrows and mouth. I can't wait to meet him some day!

Continued exhibit of a big miracle is the second picture of our dear niece Olivia. She's growing leaps and bounds, it seems like, and the way she really stares and observes people is uncanny (or so I hear). Look at those big eyes and pouty lips! I just wish I could touch her plump cheeks.

Next up: A small miracle. There are a lot of those. This year, since I wasn't feeling so hot, we were doing an ever-so-trendy stay-cation at home. I haven't even made it to downtown Seattle. But there are miracles you encounter precisely because you are spending your time slowly.

Our ancient camera's picture is not the best, but the third picture is the proof of our precious memory this summer! This spring I saw a hummingbird fly by, and in a faint hope one might come to our balcony, I started growing a pot of fuchsia. (Turns out there are huge bushels of fuchsia by the mall next door and everywhere in between, so the chances are slim a hummingbird will come to our meager one-pot wonder.)

In an attempt to feel summer-y despite persistently feeling horrid, Daniel and I did a lot of "Al Fresco!" dinners on our balcony. (It basically consists of eating our regular meals on our balcony and watching the sky and the birds and people who go by.)

One day we were having our Al Fresco dinner, and I noticed: One of the fuchsia flower looked like it was about to burst open. I pointed it out to Daniel, and at that moment, a petal went "pop!" (OK, so there wasn't really a sound, but it really felt like it went "pop!") Over the next five minutes or so, we blissfully watched other petals pop open one by one. By the end of the dinner, the flower was in its full glory. It was amazing. I mean, how many people get to witness a beautiful flower pop open, petal by petal, as it happens, with your one and only?

That was a miracle. We couldn't stop smiling that night. Such an experience probably wouldn't have existed if we had been jetting around, taking vacations like busy bees.

Over the past couple of years, we've shared a lot of emotions. Some heavy, some light, some painful, some delightful. I think we are very lucky to have done so. There have been experiences we couldn't have had if we had led "normal" lives, and our relationship is richer for that.

The rain is calming down... Maybe I'll challenge this thing called sleeping again :)

I can't be chipper all of the time - いつも前向きでいるのは難しい。

(From now on, I'm not going to note my mood or energy level or anything when I write in the middle of the night - since it's bound to be not great.)
The nighttime, coupled with pain, is such an effective invitation for dark thinking.

My logical side tells me that things will be better in the morning, yet, my heart has a hard time listening to it. Everyone else (well, except for others with insomnia or similar problems) is fast asleep, and I hear an ambulance in the distance. I hope whoever needed the care is OK.

My feet are throbbing in pain with each heartbeat I feel like I have extra hearts in there. I casually mentioned to my massage therapist yesterday that the circulation in my feet felt congested and made me feel nauseous; he immediately offered handstand as a solution. Hmm, how would I do that when my hands and wrists and arms and shoulders are in pain as well? (I know he meant well. I do elevate my feet with multiple pillows.)

My emotional side is strapped down with guilt. The guilt that I hurt. the guilt that I need to go see so many doctors and spend money on medicine. The guilt I can't do more around the house. The guilt I haven't worked for so long. (My common sense tells me that if I were a hiring manager, I wouldn't want to hire someone who gets exhausted, sweaty, light-headed and confused after 1-2 hours.) I even tried listening to meditation tapes specifically designed to help me forgive myself, but it was no use.

Then my doubts for my medical care pop up. As much as I like my doctors and they are supposed to be among the best, everyone is eager to fit me into the mold of their practice. They all promise an improvement, but often seem perplexed about my case. Just how much improvement? There is no telling, and I think a lot of it is up to me. The best I can do is feed myself well, exercise, stay optimistic, sleep, and hope I would boost things like my natural killer cells.

So I know in my head, this nighttime exercise of beating myself up is really in vain. It's more difficult for my heart to understand. I truly hope I can look back at this in the future and laugh at myself. :-/

(夜中に体調とか気分とか書いても悪いに決まってるので、
これから書かないことに決めた。)

夜中の暗さと痛みというのは、くらーいことを考え出すのに
最高のカクテルである。

自分の中の論理的な声は、「朝になればまた気分も変わるよ」と
言うのだが、これになかなか心が聞き耳を持たない。
他のひとはみんな(不眠症のひとや似たような状況のひとは
別として。でも猫も含む)寝息をたてて寝ており、
起きているわたしには遠く救急車のサイレンなぞ聞こえる。
急患のひとが大丈夫になるといいなあ、と思う。

足はうっ血しているような感触で、ずきずき鼓動とともに
痛く、まるで足に心臓があるように感じる。
こんなときは悪心で眠れないので、
昨日マッサージセラピストのジョナサンにそれとなく
自分でこんなときどうセルフケアできるか聞いて
みたのだが、「逆立ちしたらどうかな」と言われて
ガックリ。 手も手首も腕も肩も痛いというのに
どないしろっちゅーねん。
(悪気がないのはわかってるけど。
幾つもの枕で足はたかーくしている。)

感情的な声のほうは、罪悪感でいっぱい。
痛いことに罪悪感。 医者にいろいろ通って
医療と薬にお金がかかることに罪悪感。
家事をもっとてきぱきできないことに罪悪感。
(もともと得意なほうじゃないけど。)
ずっと働いていないことに罪悪感。
(常識的に考えて、わたしが雇用主だったら、
1~2時間働いただけで疲れ果てて汗を噴き出して
目まいがして混乱する従業員はほしくないと思う。)
苦し紛れに「自分を受け入れられるようになるめい想」
とかいうテープも聞いてみたけれどダメ。

で、自分は出来ることを全てしているのか、
受けている治療に対して疑問がわいてくる。
かかっている医師はみんなその道で「出来る」と言われる
人達で気に入ってはいるけれど、彼女らの行う治療は
どうしてもそのクリニックでスタンダードとなっていることに
偏る。 改善する筈、と言われるのだが、それと同時に
「もっと良くなるはずなんだけどなあ、今度はどうしよう」と
いうような顔も見せる。 どれだけ改善できるのだろうか。
それはわからない。 わたしが自分ですること、
またとる姿勢も大きく影響するのだろう。
いま出来る最善のことは、栄養をバランス良く摂って、
適度な運動をし、希望を持ち、眠って、
ナチュラルキラー細胞などを増やす努力をすること。

だから、こんな悩んでいることは役に立たないことだと
頭では分かっている。 心に悟らせるのは難しい。

こんなことを書いていたのをあとで見て、ハハハ、
こんな時もあったっけね、と笑えるようになりたいものだ。

Becoming a bouncy ball - スーパーボールになろう


It's about 3 in the morning. I woke up with screaming pain everywhere and can't possibly sleep - I was very much looking forward to going on a Safeco Field Tour with my parents today if possible, but may not make it. Sad.

I subscribe to an email service that sends out quotes as "Daily Thought." (Some of them can be silly, but hey, different strokes for different folks.) Yesterday I got this:

Cool. If that's the case, (while I'm sure there's a further bottom than this in this big world of ours and my plight is really not that bad) I'll visualize myself as a bouncy ball (a pretty purple one), but maybe even better. Instead of keeping up to 90% of its kinetic energy, I'll strive to come up even higher.

午前3時過ぎ、シアトルでは涼しい朝です。
あちこちとんでもなく痛くなって目が覚めて、眠れたものではありません。

今日は出来たら両親とセーフコ・フィールド場内ツアーに同行したかった
のですが、無理かも。 くすん。

ものによってはためになるかと思い、メールで毎日名言
(・・・ときにはしょーもないものもありますが、様々な人が
名言と感じ入るものは色々違いますからねー)を着信するサービスに
入ってるのですが、昨日来たのはこんなものでした。

「成功というものは、奈落の底まで行ってから、
どれだけ跳ね返って来るかで決まる」 (ジョージ・スミス・パットン)
というような感じでしょうか、日本語で言うと。

(まだまだこの程度で奈落の底ではありませんが)そうですよね。 
そうだとしたら、自分をスーパーボールだとイメトレしようっと。
(紫の可愛いやつ!)
で、スーパーボールなら運動エネルギーの90%とか維持する
ところ、前よりもっと高く跳ね返れるように頑張ります。

Oops, I sank again ♪ - ちょっと水没。

Weather: Clear; 61°F - still in the middle of the night, though
Energy Level: 3 out of 10
Mood: Yo-yo'ing between good and discouraged
Health: Almost out of the water? ...My lymph nodes still feel like quail eggs.


Dealing with immunodeficiency and trying to fight chronic infections (Epstein-Barr, cytomegalovirus), I feel a bit like a fishing bobber.

I think overall I'm doing a lot better compared to a few months ago. Especially according to Daniel, I seem to be able to do a lot more. I agree.

Becoming more functional means taking on more, though, and I still don't feel like I'm out of the water. I'm bobbing along. I'm not trying to catch a big fish here by any means - just little things like getting our car's emission tested (the deadline was coming up!), going grocery shopping, and cooking a dinner for my loving hubby.

Then I sink a bit. My feet and throat hurt too much to sleep.

Maybe if I can sleep some, I can come back up. Or I can get used to being in the water and become a yellow submarine - because, you know, we all live in our yellow submarine.

免疫不全の為いくつかの慢性感染症(EBウイルス性単核症、
サイトメガロウイルス感染症)から抜け出そうとしている今、
ちょっと釣りの浮きにでもなったような気持ちです。

相対的に見て、数ヶ月前からすればだいぶ元気になりました。
特にダニエルによると、前よりずっといろいろなことが出来る、と。
私もそう思います。

しかし活動できるようになったということは、いろいろしたくなる、
するようになるという事なので、それで疲れてしまい
水から完全に上がった気持ちにはなれません。
ぷかぷか半分浮いてる状態。

別に大きな魚を獲ろうとしている訳ではないのですが。
車の排気ガス点検に行ったり(期限が迫っていた)、
お買い物に出て優しい旦那様のためにご飯を作ろうとしただけ。

すると、ちょっとゴボゴボ沈んでしまいます。 
足と喉が痛くて眠れなーい。

少し眠れたらまた水上に出て来られると思います。
それか、水中に慣れてイエロー・サブマリンになるかね~♪

The skinny on Lyrica - ライリカについての情報

I love the Seattle sky when it’s cloudy like today, about to rain with moments notice. I find this kind of weather calming and soothing. Maybe it’s all the different shades of green we can see – and we do have lots of green around here despite living in a city. (Once I was touring the famed Portland Japanese Garden in rain – I personally believe this one is the best Japanese garden in the U.S., supported by the fierce volunteer corps – and the volunteer tour guide said, “You are lucky! You can see more subtle shades of green on a rainy day than a sunny day; it gives the Garden that much more depth.” It’s very true. Green is often too washed out on a sunny day.)

Well my musings aside, when it was recently approved I'd said I would ask my doctor about Lyrica, and I did, so here's the skinny from Dr. Marti, who is at Seattle Fibromyalgia & Fatigue Center.

Me: "So, have you been getting a lot of questions about Lyrica?"
Dr. Marti: "Oh yes, of course - it's the *first* approved medicine!" (With her hands swinging up in the air.)

So yes, those in-the-know do know. What she said is basically this: it's not a miracle drug, and it doesn't work for everyone. She has used it on some patients in the past (even before approval, that is), and while it does help some patients, it's not typically her first choice. The reason is that one of its primary side effects is edema, or water retention, and a lot of FMS patients have thyroid dysfunction which also causes edema. So it could kind of add to the problem in a lot of cases. Another common side effect is constipation, which can also be a problem with many patients with GI or thyroid issues (like me!).

That said, it does help promote deep sleep, so if the patient is not having the above problems and is having trouble sleeping, it could be a good fit. It does address pain but would not correct any underlying issues, so the treatment plan needs to be comprehensive to address the unique etiology of the patient.

気分: けっこうご機嫌。

今日はいかにもシアトルらしい、今にも泣きそうなはな曇りの日。
個人的にこういう日は気分が落ち着いて好きです。
光が弱いお陰で、木々の微妙な緑色が見えるせいかも知れません。
幸運なことに、けっこう都会(?)に住んでいるにも関わらず、
まわりに緑が多いのでそれに癒されます。

(昔一度雨の日に、アメリカにある日本庭園の中では多分ぴか一の
ポートランドにある日本庭園をまわったことがあるのですが、そのとき
ボランティアのガイドさんは「今日はラッキーですよ! 雨の日は、
晴れの日に見られない微妙な緑色がいろいろ見られるから、それだけ
お庭を鑑賞できる度合いが増します。」とおっしゃいました。 
ポジでいいなあ。 でもほんとだと思います。 それにこの辺出身の人は
雨が好きだし、降っても傘もささず気にしませんね。)


そんな独り言はともかくとして、以前ライリカが認可された際に
こんどお医者さんに聞いてみまーす、と言ったので、聞いてきました。
これはちなみにシアトルにある線維筋痛症・慢性疲労症候群クリニックの
マーティ先生のお答えです。

あ: 「ところで、ライリカについて沢山質問が飛び交ってますか?」
マーティ先生: 「もちろん。 だって、一番最初に認可されたんですからね!」
(みんな大騒ぎ~、と言うべく両手を挙げてひらひらさせつつ)

そう、その辺のお医者さんは知らなくても、知る人は知っているのです。 
彼女が言うには、やはり「治る」という特効薬ではないし、場合によっては
不適切な処方になることもあるので、注意が必要ということ。
認可される前も彼女は自分の患者に処方したことがあるが、
一番最初に検討する薬ではないこと。

というのも、ライリカの一番よくある副作用の一つは浮腫、またはむくれる
ことだから。 FMS患者の多くは甲状腺機能が異常になっていることが多く、
そのせいで体がすでにむくれ気味になってしまっている人が多いので、
それに輪をかけるような薬は不向きということです。 (例えば私には不向き。)
また、甲状腺機能のかげんで食べ物の消化がうまくいっていないときに、
ライリカの副作用で便秘がひどくなってしまう患者も多いとのこと。

とは言え、人によっては深い睡眠をとる助けになるので、
患者によってはいい薬になる場合もある、ということです。
ただ痛みは軽減するにしろ、体がどうして悪くなったかの根本的な原因を
解決する薬ではないので、それだけでは治療になりません。
身体機能がくるった要因は患者それぞれに違うので、それを治していく
総体的な治療のなかで、人によっては補助的な薬になり得る、ということです。

雲の合間からちょっとだけ日が射してきました。
今日は虹が見られるかも知れません。

I'm alive and (slept) well - 生きてます。。。

↑ Since I'm bad at keeping a "health journal" that my doctor recommends to keep, I thought I'd keep records here.

Someone wondered if I was alive, since I hadn't posted in a while. I am alive. I had a bit of a flare-up - this week has been tough. I woke up earlier this week feeling like I got run over by a truck, and then my menace, Mr. (Ms.?) Migraine had shown up, threatening to split my skull. I did learn from the booklet they gave us at FMS Research Program that I can't take relapses/flare-ups personally as failures, so I remained hopeful.

I had recovered somewhat by Thursday afternoon, and a good night sleep last night did me a world of good. Sleep is so important although it's hard to get for us FMS sufferers.

I picked up a good tip from a friend, which I'd like to share: have your melatonin pills by your bed, and when you wake up too early in the morning, take another one.

I take these doctor-prescribed melatonin supplement pills every night, which contain Melatonin, GABA, Valerian (root) extract, Scullcap (leaf), Passion flower (aerial), Chamomile (flower), L-Theanine, and 5-HTP. I'm told to take anywhere between 2-6, but since I want to take as little as possible, I've been trying to only take 2-3 (equaling .5 mg of Melatonin). It didn't seem to matter how many I took though, because with little correlation to the dosage I kept waking up way before sunrise, after I'd go to sleep at 2 AM. Last night I took a couple more at 4:30 AM when I woke up - and I was able to go back to sleep! Yay. Thank you, Gwynn!

So instead of 1 out of 10 (not able to get up), I'm at 3 out of 10. I'll take any gain I can get.

↑ お医者さんが健康日誌をつけると良いですよーとおっしゃっていたのですが、
日記をつけるのはなにぶん苦手なのでここに記録してみます。


最近書き込みがなかったため「おーい生きてる?」と聞かれましたが、
おかげさまで生きております。
今週はちょっとつらい週でした。
ある日トラックに轢かれたような(轢かれたことないけど)感じで
起きて、その後下り坂、しかも宿敵の偏頭痛さんが出現!
頭が割れるかと思った。

でも症状の突発を自分の不出来・失敗と思って落ち込んではいけない、と
ちけんグループの勉強会で学んだので、ポジに生きていくことにしました。

幸い木曜午後にはだんだん頭痛が薄れて来、
ゆうべやっと眠れたおかげで今日は少し元気です。
睡眠って本当に大事ですね~。
でも他のFMS患者の皆さんと同様、
一晩眠るのは難しいです。

知り合いの方からいいヒントを教えて頂いたので、
ここに書きとめておきます。
寝床のそばにメラトニンの錠剤を置いておいて、
夜や早朝起きてしまったときに駄目押しの一~二錠
飲むとまた眠れる場合が多いということ。

私の場合、お医者様にもらったメラトニンの入ったサプリ錠剤を
毎晩飲むのですが(メラトニン、GABA(ガンマアミノ酸)、
吉草根(カノコソウ)、スカルキャップ、トケイソウ、カモミール、L-テアニン、
5-HTP(L-5水酸化トリプトファン)が入っています)、
2~6錠飲むようにと言われたものの、出来るだけ頼りたくないので
最近は2~3錠だけ飲むようにしていました。 
(3錠でメラトニン0.5mgになります。)
あまり服用量に関係なく、2時ごろ寝ても夜明けにどうしても
目が覚めてしまっていたからです。

そこでお友達の言った通り、明け方起きてしまったときに
数錠飲んでみたところ、本当に眠れたのです。 
やった~。 (グウェンありがとう!)

ということで、10点満点のうち1の元気度(起き上がれない~)が、
3ぐらいに上がりました。
上昇する限り、いくら少しの違いでも嬉しいです。

The tortoise and the hare - ウサギと亀

Woke up after almost exactly 2 hours of sleep.


It was a horrid dream and muscle spasms that woke me up. I was dreaming I was trying to reach for my topical medication for pain, which I needed in reality. In the dream I had taken some bad combination of drugs which made debilitating numbness trickle down from my head, slowly taking over my whole body. To make matters worse, Daniel next to me was expressing some physical change of his own - it was contagious!

Sometimes our worst fears subconsciously creep into our dreams.

I'd watched my ex-mother-in-law through her last stages of MS. At the end, she was paralyzed from her neck down, with very little control over her fingers on just one hand. But cruelly, pain and discomfort remained. Prevalence of MS in Colorado, where we lived, happens to be higher than usual for some unknown reason, just like Washington state; so my ex was always worried that I might get it one day.

Sharon was a proud woman who'd put herself through school in her middle age and landed herself a good career before she became bed bound. I loved her spirit and looked up to her. She was active all her life - she loved riding horses and hiking; she bowled in a league and loved playing softball. When her knee buckled and her leg went limp for the first time, she was running to the first base.

She hated to be helpless and it drove her mad she had to ask for help from others. She was used to being in charge; she was a caretaker of others; she was the dependable one. The fact she depended on us ate her away.

While my illnesses are not even close to MS in terms of being life threatening (as FMS by itself almost never kills and celiac disease is almost always treatable when caught early enough), that helplessness must be where my fears lie.

I was used to taking care of myself and others; I was used to being in charge. I started out as a weak child, but I worked hard at becoming stronger, making myself a leader in different teams. I hate the fact that I'm now the weak link.

When I was looking after my mother-in-law, I wished she'd be less fussy about being helped, and now I know exactly how she felt. Helpless. I feel like a wind can blow and make it so that I suddenly can't walk further.

I participated in different forms of athletics all my life, and I tend to subscribe to "no pain, no gain" philosophy. The fear of becoming helpless was driving me to walk further, to maybe overdo the exercises for the fibromyalgia study when my body was still recovering from EBV/mono infection. Hence the pain that disturbs sleep.

I'm learning. I need to back off myself. I need to learn to be a tortoise.

...And there goes the birds.

P.S. Thanks to the rainy weather, I was able to go back to sleep some more. :-) Maybe I'll take it easy today.

2時間だけ寝たあと、ばっちり目が覚めてしまいました。

悪夢と痛み、けいれんのせい。
夢の中で、痛み止めと抗けいれんに効果のある塗り薬をとろうとするのですが、
何か良くない薬の飲み合わせのせいで、頭の先からだんだん痺れて、
体が麻痺していくのです。しかもそれは何故かうつってしまい、
隣に寝ているダニエルもなんだか体の変化を訴える。

きっと、自分が今一番恐れていることが、
意識下から夢に忍び込むのですね。

以前結婚していたとき、義理の母がMS(多発性硬化症)だったのを
看病してその影響を目の当たりにしました。

彼女の場合は首から下ほとんど全部麻痺してしまい、
最期には何本の指かだけときたま思うように動かせるだけ。
でも残酷なことに、痛みや痺れなどの不快感は続く毎日でした。

何故か前住んでいたコロラド州ではMSの発症例が他より多く
(ワシントン州もそうですが)、前の旦那が私もいつか発症するんじゃと
おどおどしていたのを覚えています。

義母のシャロンは誇り高い女性でした。
子育てをしている間に少しずつ大学・大学院へと通い、
いい仕事を勝ち取ったあとの発病。
強い精神の持ち主で、どうやって生きてきたか話すたび
お手本にしたいと思いました。

病気をする前は生涯通じて非常にアクティブで、
乗馬やハイキングを愛し、リーグに入って
ボウリングとソフトボールを楽しんでいました。
40代はじめ、ソフトボールの試合で1塁に走ろうとしていた最中、
突然膝ががくんと落ち脚がだるくなって走れなくなったとのことでした。

彼女はひとに弱みを見せるのが嫌いで、
周りに助けを乞うことがいやな性格でした。
周りの世話役として、しっかり者として人生を過ごしてきたからです。
私たちに寄りかかざるを得ないことがとてもつらいようでした。

私の病気はMSのように命に関わるものではないので、
彼女のとてつもない不安とは比べようがありません。
(FMSは治らなくてもそれだけで死に至ることはありませんし、
セリアック病も早期発見すればがんなどに発展することはなく、
グルテンフリーの生活を続ける限り、また悪化はしません。)
でも、同じような無力感が私の恐れていることなのだと思います。

シャロンほどのしっかり者ではありませんが、
自分の身の回りのことは(ほどほどに)自分でしてきて、
周りに気を配り世話焼きをする側で行動してきたからです。
小さい頃は細くて体もそんなに強くなかったのですが、
頑張って努力して、スポーツではチームキャプテンなどにもなれました。

それだけに、こうして今「弱い者」になったのが悔しいのです。

義母を看病しているときは、「もうしょうがないんだから、
素直にお世話させてくれればいいのに」と思いましたが、
(残念なことに)彼女がこの世にいない今、
気持ちが痛いほど分かります。 

無力感、弱者になった気分。
風がふうっと気まぐれで吹けば
痛くなって歩けなくなるかもしれない、と恐れること。

体育会系のクラブに所属し、学校以外でも
スポーツを続けてきた私は、「痛くなければ強くならない」と思ってきました。

無力に、弱者になるのを恐れて、そのせいで
リサーチに参加する上での運動を、少しやりすぎたのかも知れません。
EBウィルスとサイトメガロウイルスに感染して単核症になっていたのに、
それを乗り越えようとするあまりプッシュし続けたのが
裏目に出たのかも。 だからきっとやってくる、眠りを防ぐ痛み。

アホですが、学習しています。 ちょっと引くこと。
良くなるためには亀になること。

はぁ、また鳥がさえずってきました。

When a birdlover goes postal - 鳥好きが切れるとき

In the 1988 comedy Funny Farm, Andy, a city-slicker writer played by Chevy Chase, realizes his dream of becoming a novelist and owning a home in the country. The home even comes with a charming songbird who perches at his window and keeps him company where he sets up his typewriter. Upon moving in, everything seems so charming and perfect.

Later, there's a scene in which Andy, after struggling with writer's block for weeks or months and listening to the bird, blankly stares at his feathery friend and dumps coffee on him.

I felt a bit like Andy today.

We live on the 3rd floor of our building with our windows facing some lovely big trees. To us, that was one of the selling points. We love big trees. We love birds. Even crows, with their clever social behavior, charm us. I would set up a bird feeder and a birdbath on our balcony if our HOA weren't so Nazi about it.

It's harder to love them at 4:30 AM, when you could only fall asleep past 1 AM and lovely bird chirps wake you up 2 days out of 3.

According to my FMS doctor Dr. Marti (who is wonderful to work with), getting long sleep is a major component of effective care in fibromyalgia. Patients tend to be going up against a deficient immune system, especially during a flare-up, so it's crucial to let your body sleep for prolonged amounts of time. This helps bodies mend themselves, building a stronger immune system and helping correct the screwed-up hormone imbalance including adrenal and thyroid dysfunction, both of which come along with FMS. Sleep is amazing.

When not getting enough sleep, the body is grumpier. Can't make enough cortisol to get through the day, which makes us so tired. It also makes us susceptible to secondary infections and other conditions.

It sounds so simple. But when you are in this vicious cycle (tired/hurts→can't sleep→tired/hurts more→can't sleep), getting sleep is easier said than done. It's hard to sleep through pain and overwhelming fatigue. Often it hurts just to lay down - we've invested in a new bed base, new mattress, heated mattress pad and 5 new pillows so far but still can't stay asleep every day.

And that's why I feel like shooting these birds with a water gun today. Like Andy.

On the plus side, it's another beautiful day in Seattle! :-) Maybe bird moms/dads were saying, "Today, I'll get you breakfast and lunch and dinner AND dessert!" I hope the birdies have a great day blessed with lots of bugs.

1988年に公開された ファニー・ファームというコメディー映画があります。

コメディアン チェビー・チェイス扮するアンディは、
「田舎に住んで本を書くのに没頭する」という決心をして夢をかなえた記者。
田舎に買ったマイホームには、チャーミングなことに
毎日窓に訪ねて来てさえずってくれる小鳥までついてきます。

引っ越した当初は何もかもばら色で、チャーミングに見えます。
しかし毎日、その窓の隣に置いたタイプライターの前で、
筆が止まってしまうアンディ。 

何週間か何ヶ月後かのとある日、
一日も欠くことなく窓のふちに来て歌う鳥を前にして、
アンディは一瞬うつらな目つきで考え、コーヒーをぶっかけるのです。

今日はそんな気分になりました。

うちは3階のマンションなのですが、周りにある
大きな常緑樹に囲まれています。 家を選ぶとき、
それはプラスポイントでした。 私達は大きい木も、
鳥も好きだから。 鳥にいたっては嫌われ者の
カラスも好きです、賢くて見るのが楽しいから。
住民会が厳しくなければ、小鳥の水浴び場とか、
餌箱をベランダに置きたいところです。

でも3日のうち2日は眠れず、午前1時過ぎに眠りについて
朝4時すぎに鳥の声で起きるとなると話は違ってきます。

かかりつけのFMS治療が専門の医師(とってもお話しやすいマーティ先生)
によると、線維筋痛症の治療が上手くいく為には
体の代謝を促し癒す長い睡眠が欠かせないとのこと。
患者は既に発症した時点で免疫系の機能不全になっており、
さらにその一因として副腎機能と甲状腺機能が異常になっているので、
正常なバランスを取り戻すためには長い睡眠が不可欠だとおっしゃいます。

ちゃんと眠っていないと、体は反抗します。 一日動くために必要な、
コルチゾールが生産されないので、次の日ひどい倦怠感におそわれる。
さらに普通の人だったら防げる他の病気にかかりやすくなる。 

シンプルに聞こえます。
でもこの 疲れる・痛い→眠れない→もっと疲れて痛い→もっと眠れないという
悪循環にはまってしまうと、なかなかこのシンプルなことが
できないんですね~。 痛み止めも飲み続けると効かなくなるし。

一日が終わって横になると寝るのも痛いので、
新しいベッドの土台からマットレス、ヒーター付きマットレスパッド、枕も
5個ほど新しいのを買ったのですが、苦戦中。

というわけで今朝は、鳥を撃つ水鉄砲でも欲しくなりました。
アンディのように。

いいニュースは、雨があがってまたきれいなシアトルの1日が始まること。
鳥さん達は、「今日はい~っぱい食べ物捕ってきてあげるからね~」と
はりきっていたのかも。 鳥さん達、餌取り頑張ってね~。

Art of shorter blog posts – 簡潔にする努力

Apparently I jinxed myself yesterday, because a sharp stomachache awoke me along with lovely bird chirps. With sunrise in Seattle approaching 5:00 AM this time of the year, it’s getting harder and harder to stay asleep for someone with fibromyalgia, which tends to cause disrupted sleep cycle anyway.

In the past 2 days I’ve learned that getting carried away with long typing sessions results in tendon pains the next day, as if I kneaded bread dough all afternoon. C'est la vie. :-P

This is good news for you: I’ll try to learn the art of short(er) blog posts.

In that spirit, here’s a haiku:


Fibromyalgia

口に出したのがジンクス化したのか、

早朝の小鳥のさえずりに合わせて腹痛に起こされました。


初夏、緑が美しく日が長いシアトルでは、日の出が5時に迫り、

もともと睡眠障害になり易い線維筋痛症患者としては

一晩眠り続けるのが難しくなります。


ここ2日の経験で、調子にのってキーボードをながながと使うと

次の日手と腕の筋が痛くてたまらないことを覚えました(アホ)。(^_^;)


これは皆様に朗報。 もう少しポストを簡潔にするよう努めます。
